Keeping the Promise of Hoag CARES Palliative Services

Palliative care is kindness in action. An emerging specialty, palliative care is not yet commonplace. Yet, it plays a key role in the continuum of care. As a leader providing care across the spectrum, Hoag is proactively advancing the field of palliative care. Not surprisingly, donor support allows us to provide breakthroughs that minimize suffering and enhance quality of life.

 

With donor support, Hoag is building a robust and leading-edge palliative care program that serves as a model for others nationwide.

Big Ideas

The Hoag Promise Campaign will invest in advances in technology and new approaches to care to reduce suffering and increase quality of life for our patients.

  • Ambulatory CARES Program - Facilitated by an advanced practice nurse, this comprehensive program will include home visitations and clinic care for seriously ill patients allowing improved symptom management and quality of life.
  • Research and Quality Improvement Projects - Palliative Care Quality Network membership and publications provide the opportunity to conduct, publish and present research establishing best practices in palliative care.
  • Electronic Medical Records - Build electronic medical record capable of interfacing with primary treatment teams to facilitate comprehensive, best practice palliative care.

Empowering You

The Hoag Promise Campaign priorities empower patients with the right resources, tools and information, at the right time.

  • Symptom and Quality of Life Navigator – Navigation by a Clinical Nurse Specialist including symptom management, transitions across the care continuum and on-going patient, family and staff education around chronic conditions and palliative care principles to improve quality of life.
  • Comprehensive Bereavement Program - provides the families of patients who have died in the hospital with on-going support in the form of printed materials, telephonic and in person counseling sessions with a Licensed Clinical Social Worker and group bereavement therapy.
  • Spiritual CARES Program - Comprehensive spiritual care provided by a dedicated chaplain to assist in delivering patient centered care. May facilitate improved goals of care discussions as patients are able to explore religious and spiritual meaning associated with medical decision making.
  • Patient and family Support Program - Provide on-going education and support to patients and families dealing with serious illness, including provision for clinical staff to facilitate psycho-educational support groups to patients and family members.

We are extremely fortunate that our community has attracted physician and nurse leaders of the highest caliber, dedicating their careers to saving lives. Here’s how the Hoag Promise Campaign will support them in their important work.

  • Physician Fellowship - Educate future physicians in palliative care medicine techniques and establish Hoag as a national training ground for palliative care best practices.
  • Clinical and Community Education Programs – Palliative care education throughout the community, including physician offices, skilled nursing facilities and community partners on subjects such as advance care planning, working with distinct care populations and the benefits of early palliative care.
